REQUESTS FOR PROPOSAL VILLAGE OF CLEARWATER

Old ambulance Bay HVAC

The Village of Clearwater is seeking Requests for Proposal from experienced and well-qualified contractors to install an HVAC system in the old ambulance bay at 626 Main St., Clearwater. A qualified professional contractor shall have experience in projects of similar scope and size as described in the scope of services section. A contractor may bid all or part of the project.

The selected contractor(s) shall furnish all equipment and materials to the specifications of the village and library boards and will be responsible for applying and paying for all necessary permits. A Form W-9 and certificate of liability insurance shall be provided to the village by all contractors and subcontractors before work begins. Invoices must be submitted to the village clerk to present to the village board at their regular meetings. Payments will not be made until board approval.

SCOPE OF SERVICES AND CRITERIA

The scope of the project shall include:

• Remove unnecessary heaters along interior ambulance bay walls and ceiling. Dispose of inoperable heater and other abandoned HVAC equipment.

• Replace with heating and air conditioning HVAC system sufficient for ambulance bay and telehealth rooms. Specs shall be approximately one two-ton thirteen-seer air conditioner, one single-stage 80,000 BTU gas furnace and one two-ton evaporator coil.

• Electrical wiring as required to install new system including thermostat.

SELECTION PROCESS

The final selection of the contractor will be according to the following:

1. Technical expertise of the contractor in connection with the proposed scope of services to be provided. (40 points)

2. Capacity of the contractor to perform the work within time limitations, taking into consideration the current and planned workload of the contractor. Include a project team for this specific job and qualifications of all team members. (30 points)

3. Proposed cost and fee schedule. (30 points)

Total possible points, 100 points

Elaborate and expensive presentation aids are neither necessary nor encouraged. After an initial review, interviews may be conducted followed by a rescoring of proposals. The contractor’s proposal that best meets the needs of the village will be considered. Negotiations of the contractor’s proposed scope of services and contract amount may be conducted after the selection of the respondent’s proposal.
